Gearing up for the Fall Vaccine Rollout

As we roll through September, we begin gearing up for respiratory virus season.

Manitoba Health is recommending that all long-term care residents receive both a Seasonal Influenza Vaccine and the Covid19 vaccine from Moderna. The seasonal flu vaccine will be the “high dose” for residents who are over 65. The Covid19 vaccine being offered will be the recently approved Moderna “Spikevax” formulation specific to omicron XBB.1.5

To be proactive, we have included two fact sheets and the Consent form that the designated Legal Decision Maker for your loved one may complete and return to us.  More specifically, it would be tremendously helpful to us if you are able, to:

  1. Print the Consent form (it is a two-page document that can be printed double-sided or on two separate pages), and complete section A (you need only fill-in your loved one’s Surname and Given Names) and section D 1. Consent by parent or legal decision maker (including selecting the vaccine(s) you wish your loved one to receive) ; and
  2. Drop off the completed Consent form to our Business/Administration office. After hours and/or on weekends, please use our “mailbox slot” next to the Business office door for secure drop off purposes.

The form may also be completed online/digitally and emailed to Justin Lagace at .

If you have any specific questions regarding the Consent form or the vaccines, please connect with Justin at 204 453-4663 ext. 206 or by email at:  . Should we not receive your response in the coming days, a member of our team will reach out by phone to discuss.

We are most appreciative of your assistance in the completion/return of the Consent form. Working together always makes the overall process that much smoother and efficient.

Let’s make both the “sweater weather” and “snow boots” seasons health-safe for you and our Residents!  Keep Well!

Warmest regards,
