Stepping Into September – Getting Ready for our Annual Flu Shots

We are starting our preparations for the Fall Influenza Vaccine campaign, and we need your help.

New for 2022, we are reaching out to you (specifically our Residents’ designated primary contact) to share the Flu Vaccine Fact Sheet from Manitoba Health. Please take a few moments to review the Sheet to ensure you are well-informed about the vaccine. Also new, is the Influenza Consent form that must be completed/signed.

To help us get a jumpstart on the overall paperwork process, we would be most grateful for your direct involvement/assistance by:

  • Printing the Consent form, and completing section A (at minimum, please complete with your loved one’s Surname and Given Names) and section D 1. Consent by parent or legal decision maker; and
  • Dropping off the completed Consent form to our Business/Administration office prior to September 10. After hours and/or on weekends, please use our “mailbox slot” next to the Business office door for secure drop off purposes.

Please note that all Residents 65 years and older are eligible for the High-dose Influenza Vaccine.

For those who do not have access to a printer, you may request a copy of the form from any of our nurses when next visiting at The Home.

Should we not receive the completed Consent form for your loved one by/on September 10, we will commence calling families (i.e., the primary contact for the Resident) to seek verbal consent for the influenza vaccine.

Many have also been inquiring about the Covid19 omicron specific mRNA booster and we anticipate receiving further information on the rollout of this booster in the near future. As soon as we know – we commit to sharing with you!

If you have any specific questions regarding the Consent form or the flu shot, please connect with Justin Lagace, RN, at 204 453-4663 ext. 206 or email him at:  .

We are most appreciative of your assistance in the completion/return of the influenza Consent form. A little teamwork will make the overall process that much smoother and efficient.

Together, we can make the Fall and winter seasons health-safe for you and our Residents!

Warmest regards,

Sherry Heppner
